Step 2 — Your family tells us who they are. Before any plan is written, we send your family our Client Intake Form — a “getting to know you” document that goes far beyond medical history. Her story. Our founder personally reads every intake form — because a care plan built without knowing the person isn't a care plan.
Step 3 — RN in-home assessment. A registered nurse visits your loved one at home — often the same day — assessing care needs, daily routines, and running a full in-home safety check. Hospital discharge pending? We coordinate directly with the discharge planner.
Step 4 — Your written care plan. Together we determine the services and hours that actually fit — from a few hours a week to 24/7 coverage. You see the plan in writing before anything begins.
Step 5 — Thoughtful caregiver matching. Using your intake form and the RN assessment, our founder personally shapes the concierge care plan and hand-selects the caregiver team — Maryland-certified CNA 1, CNA 2, and LPN caregivers matched to your loved one's needs, interests, and personality.
Step 6 — Care begins; family stays connected. Your family follows schedules, caregivers, timesheets, and invoices in the app — and every visit is recorded in your loved one's personalized care notebook, reviewed by a registered nurse. Photos and live calls come directly from your care team.
Step 7 — We keep adjusting. A registered nurse formally reviews the plan every 90 days — or immediately when needs change. If LTC insurance is involved, we prepare and file the reimbursement paperwork for you. Over 98% of our clients' claims are successfully reimbursed.
